Mother, 37, died giving birth to twins 'after a hospital emergency button failed, medics recorded her blood type incorrectly and specialist fluids were given to the WRONG patient'


Estelle O'Sullivan (pictured left), 37, lost three litres of blood after the Caesarian operation to deliver her twins and later died from a cardiac arrest at Wexham Park Hospital in Slough (right), Berkshire. 


A coroner heard that an emergency button in the operating theatre did not work, leaving a midwife to dash through hospital corridors to get help when things went wrong. Medics had also written down the wrong type of blood on her records and specialist fluids ordered to be delivered to the hospital were given to the wrong patient. Ms O'Sullivan had been a patient at Wexham Park Hospital in Slough, Berkshire, when the tragedy happened, the inquest heard, and apart from the blood blunders, essential operating instruments were missing from the theatre.
